In order to help make your visit a convenient and pleasant experience, please print out a copy of the following forms to fill out and bring with you to your first appointment:
• Initial Podiatric History
• Financial Policy
In addition, please bring the following:
• Written Referral, If required by your Insurance
• Medical Insurance and ID Card
• Previous x-rays and medical records, if necessary
• Shoes
Please be prepared to pay for the following at the time of your visit:
• Co-Payment, if applicable
• Office supplies not covered by insurance
• Full cost of vist if no insurance
